Care Management

To every senior in need, ECS asks the question, "What do you need to make your life better; to help you be more independent, safer and healthier?" A Care Manager will visit you or the person needing help and together, will create a comprehensive care management plan and decide what services are needed.

Services available include:

• In-Home Direct Care Services such as light housekeeping, personal care, and companionship.
• Transportation to medical appointments, pharmacy and grocery store through our S.T.A.R.S. program.
• Elder Day Stay is a home away from home for seniors who can no longer be left alone due to physical or mental disabilities.
• Home delivered meals are available through Meals on Wheels.
• Alzheimer’s and General Respite Care is available to caregivers.
• Legal Assistance
• Medical Management
• Emergency Assistance
• End of Life Issues

Care Managers will also provide information and referral and explore community resources, including assisted living facilities and nursing homes, to meet yours or a loved one’s needs. The goal is keeping the senior as independent as possible while giving them and their families the services they need.

Elder Care Services, Inc. is a local non-profit dedicated to helping seniors live at home with dignity. Our extensive experience helps us understand your concerns and difficulties.
