CARE MANAGER

Care Management position, full-time.  Degree in social work, sociology, psychology or related field required, Masters preferred. Position responsible for assessing, planning, and supporting functionally impaired elderly persons in the community. Candidates should have basic computer skills, excellent communication skills, a working knowledge of aging issues, and professional experience with this population.  Mandatory Level 1 background check, pre-employment drug screen and TB test.  Send resume to ELDER CARE SERVICES, 2518 West Tennessee Street.  EOE.  ADA.  Drug Free Workplace.
