ELDER CARE SERVICES
 STRATEGIC PLAN

Goal 1: Elder Care Services will become the primary care manager for the majority of elders in the community who need services.

 

            Objective 1:  Care managers will pilot a comprehensive care planning process and product with 80 clients and their caregivers by August 31, 2009.

Objective 2:  The Division of Home and Community Services will develop a protocol with the Aging Resource Center to assess consumers who are on the Medicaid Waiver waiting list within 7 days of being authorized and counsel others within 14 days of their initial screening by the end of 2008.

Objective 3:   Elder Care will establish a care management relationship with Capital Health Plan, Tallahassee Memorial Hospital and Capital Regional Medical Center by August 31, 2009.
           

 

Goal 2: Elder Care will become the recognized first source of information, education and resources to seniors, their caregivers and the community.

 

            Objective 1: Information and referral will be provided to all clients who call or walk in.

Objective 2: All clients who are referred to the Aging Resource Center or other agencies will be followed up with and encouraged to participate in a care management assessment by January 15, 2009.

Objective 3: By January 31, 2010, Elder Care will be the primary provider of educational programs on the elderly to professionals, caregivers, families and clients.

Objective 4: By July 31, 2009 Elder Care will take a community leadership role in developing new and needed services to meet clients’ needs.
           

 

Goal 3: Elder Care will become a major influence upon state and local legislative, policy and budget decisions concerning aging and senior issues.

 

            Objective 1:  During 2009, strategic relationships will be established with state and local advocacy groups, including AARP, FCOA, FASP, CCE Coalition, UPHS and the United Way.

Objective 2: During 2009, a variety of communication media and special events will be used to highlight important senior issues.

Objective 3: During 2009, Elder Care staff and board members will use personal contacts to advance the ECS agenda at the local and state levels.

Objective 4: During 2009, Elder Care will support efforts to increase funds available to meet client needs, funding that follows the client, and resist private/for profit agencies encroaching upon public funds.

 

 

Goal 4: Elder Care will operate with a balanced budget that reflects revenue sources that increase the portion of non-governmental funds to 25% by January 2010, including income from a substantial endowment.

            Objective 1: Increase non traditional grants by $20,000 each year.

Objective 2: Increase fund raising return by $25,000 each year.

Objective 3: Increase endowment contributions by $20,000 each year

Objective 4: Increase program income by $50,000 each year while controlling expenses without sacrificing service levels.

Objective 5: Establish an advisory council and marketing plan for a separately identified fee for service department within Elder Care Services.

 

Goal 5: Elder Care will open a new full service Elder Care Center by 2012 that includes offices, congregate and meeting rooms, day care and other related community agencies.

            Objective 1: Elder Care will develop a long range capital development plan for a full service center complete with alternatives, drawings, specifications and projected costs by July 31, 2009.

Objective 2:  Elder Care administrative staff will identify potential locations for a full service center to replace the current main facility by December 31, 2009.

Objective 3:  Elder Care administration and Board members will develop a fund raising plan and identify potential donors for a capital campaign by July 31, 2009.
